Who We Are

We are a research team at Yale School of Medicine, Department of Clinical & Community Research.

Our mission is to improve the quality of life for people who are involved in the criminal justice system, who are at-risk for (or living with) HIV, and are homeless, experiencing mental illness, difficulties with substance use, or need help with employment. We will connect you to medical and behavioral health services, housing, employment, and addiction treatment through a collaboration between Yale School of Medicine and Liberty Community Services.

This project is for people in New Haven who are justice-involved (recently released from prison or jail, or on probation/parole), and are experiencing homelessness. If you believe this project can benefit you, call our study number at (475) 414-7789 or (203) 499-8075 for more information.

Project CHANGE is a service project funded through the Substance Abuse and Mental Health Services Administration (SAMHSA [H79-TI080561]) Yale HIC #2000024880
