Housing & Employment Services

Liberty Community Services provides support to homeless individuals who are living with HIV/AIDS, mental illness and/or addiction. Programs include permanent supportive housing, rapid rehousing, day programs that offer meals and access to showers, case management, income and employment services, food programs, and homelessness prevention.

Medical Health Services and Referrals 

The CHCV operates Monday through Friday and provides effective outreach and intervention activities to re-engage individuals who are lost to care with its bilingual staff providing services in primary and HIV care, substance use disorders, mental health, and case management to stabilize those in crisis or in need.

HIV testing is part of routine care on the CHCV as well as other tuberculosis, hepatitis A, B, C, syphilis, gonorrhea and chlamydia.  

In addition to the CHCV, the program provides health care and harm reduction services (e.g. syringe exchange, naloxone distribution) at no cost to patients at our office location and with a minivan that travels throughout the greater New Haven area.

Behavioral Health and Addiction Services

In addition to the Community Health Care Van, the CHCV Behavioral Health Team includes social workers and psychiatric nurse practitioners who are available by appointment and specialize in substance use disorder counseling and mental health counseling. Substance use treatments may include Buprenorphine (Suboxone®) treatment, extended-release naltrexone (Vivitrol®) treatment, counseling and case management, and linkage to inpatient services and methadone maintenance therapy.
